All applicants must be the appropriate age by August 31.

Custody Forms must be filled out for all children of divorced or separated families.

Holy Family School admits students of any race, religion, color, national and ethnic origin. All Catholic families should be registered contributing parishioners of one of our five parishes: Holy Trinity, Sacred Heart, Saint Ann, Saint Joseph and Saint Mary of the Assumption. Non-Catholic students must commit themselves to the study of the Catholic religion, must fulfill all the requirements of the subject, and must attend all religious functions offered as part of the school program. Their parents must commit themselves in writing to accept and to promote the philosophy, goals, objectives and regulations of the school and assume responsibility for all financial obligations as specified by the pastoral board.

Preschool, Pre-Kindergarten, Kindergarten and First Grade and All New Students
Admission Requirements and Information

The following must be presented in order for registration to be completed:
  • Registration Form and non-refundable registration fee (PDF / Word)
  • Medical Form (PDF / Word)
  • Health History Form (PDF / Word)
  • Custody Form for children of divorced or separated families (PDF / Word)
  • A copy of your child's baptismal certificate
  • A copy of your child's birth certificate
  • Parish Approval - New Families (Please contact your parish)
  • Request for academic records (first through eighth grade applicants only)

Kindergarten Requirements

All students entering kindergarten must:

  • Meet with the school nurse during formal registration hours or by appointment.
  • Have all of the forms listed above as well as:
    • Physical Examination Form (PDF / Word)
    • Dental Examination Form (PDF / Word)

Returning Students Requirements

All returning students entering grades 1-8 in the fall must have:

  • Registration Form (PDF / Word)
  • Medical Form (PDF / Word)
  • Health History Form (PDF / Word) (only necessary if history has changed from previous year)
  • Custody Form for children of divorced or separated families (PDF / Word)

Grades 2-8 Admissions Requirements

In addition to the admission requirements listed above, acceptance in grades two through eight is conditional, pending a review of academic and behavioral records. An interview with the school principal may be a requirement of registration.
